Prostate cancer is one of the most increasing prevalence of cancers in Japan. It is well known that vitamin E acts as a useful chemo-preventive agent against several types of cancers. However there are conflicting results on a role of vitamin E in the prevention of prostate cancer. In this study, a meta-analysis of current evidence from randomized controlled trials (RCTs) was conducted to evaluate the role of vitamin E in the prevention of prostate cancer. Relevant articles in PubMed were systematically searched for RCTs of vitamin E intake and incidence of prostate cancer. From 455 potential articles, 6 eligible RCTs articles were selected. Meta-analysis of 6 RCTs identified showed no significant difference on the effect of vitamin E intake alone on the reduction in incidence of prostate cancer (Relative Risk (RR)=0.88, 95% Confidential Interval (CI): 0.71, 1.08, P=0.22; random effects). However, there was heterogeneity in this meta-analysis, so that this result suggested that vitamin E intake might have a protective effect against prostate cancer in some cases.

Haptoglobin (Hp) polymorphism has recently been demonstrated to be associated with cardiovascular risk and to impact significantly the disease-preventing and therapeutic effects of antioxidant vitamin E in disorders associated with increased oxidative stress, such as diabetes. This suggests that patient population selection based on disease type and Hp polymorphism may dramatically improve the disease-preventing and therapeutic effects of vitamin E administration.

Cytochrome P450 (CYP) 4F2 was identified as a leukotriene B_4 ω-hydroxylase, and is responsible for hepatic ω-hydroxylation of vitamin E and vitamin K_1, and renal production of 20-hydroxyeicosatetraenoic acid (20-HETE), a vasoconstrictor and natriuretic substance. Single nucleotide polymorphism (SNP) rs2108622 is a missense mutation, V433M, in CYP4F2 gene, which is of considerable clinical importance. CYP4F2 V433M polymorphism reduces production of hydroxytocopherol, hydroxyphylloquinone, and 20-HETE in vitro. SNP rs2108622 was associated with serum α-tocopherol, but not γ-tocopherol concentration, in two genome-wide association study cohorts and interindividual variability in response to coumarin drugs in systematic review and meta-analysis. The SNP also was associated with systolic and diastolic blood pressure and incident ischemic stroke in men, but not in women, in a large cohort study.

Cytokines are critical regulators in immune responses and inflammation. Vitamin E (VE), α-tocopherol, is known to improve immune function, especially in the elderly, and to exert anti-inflammatory action in part by altering the production and release of cytokines. However, several large cohort and clinical intervention studies have shown that supplemental VE does not always exert its immunomodulatory and anti-inflammatory effects in healthy people and patients with diseases such as atherosclerosis and infections in which impaired immune function and inflammation participate. As one of the reasons, it has been assumed that an individual's genetic background affects the response to VE supplementation under conditions of impaired immune function and inflammation. Recent several studies have suggested that the effect of supplemental VE on impaired immune function and inflammation is dependent on an individual's single nucleotide polymorphism at cytokine genes. In this mini review, the relationship between the effect of supplemental VE on the production and release of cytokines and polymorphism has been discussed.

Vitamin E is an important fat-soluble micronutrient for higher mammals and functions as an antioxidant for lipids and also as a regulator of gene expression and a modulator of cell signaling and proliferation. To exert its adequate functions, vitamin E should achieve an appropriate disposition throughout the body via several processes. Among factors contributing these processes, this review mainly focuses on the relationship between polymorphisms of lipoprotein-related factors and blood vitamin E level.
