    It has been shown that the liver function is closely related to the respiratory activity of the liver. We have investigated the relation between the liver respiratory activity and the glucose tolerance with respect to serum m-GOT level.
    The patients with normal glucose tolerance showed a negative correlation between the respiratory activity and the serum level of m-GOT, while the patients with abnormal glucose tolerance showed a decreased respiratory activity, and didn't show a correlation between the respiratory activity and the serum level of m-GOT.
    It is concluded that, when the glucose tolerance is normal, the serum level of m-GOT in patients with chronic hepatitis or cirrhosis reveals the liver respiratory activity.

  • Bile acid composition in fluminant hepatitis and acute hepatitis

    We studied individual bile acids in 9 patients with fluminant hepatitis (FH) and 12 patients with acute hepatitis (AH) using high performance liquid chromatography and immobilized 3α-hydroxysteroid dehydrogenase in column form (by OKUYAMA 1979).
    1) There were no difference in total bile acids (TBA), conjugation rate and glycine-conjugated bile acid/taurine-conjugated bile acid ratio (G/T ratio) between FH and AH.
    2) Percentage of glycocholic acid, taurocholic acid and cholic acid to TBA were significantly higher in AH than in FH (P<0.05). Glycocholic acid/glycochenodeoxycholic acid ratio (GC/GCDC ratio), taurocholic acid/taurochenodeoxycholic acid ratio (TC/TCDC ratio) and cholic acid/chenodeoxycholic acid ratio (C/CDC ratio) were significantly higher in AH than in FH, too (P<0.05).
    3) Percentage of cholic acid and chenodeoxycholic acid to TBA and C/CDC ratio correlated closely with Thrombotest (TT), Hepaplastintest (HPT) and α2-heat stable glycoprotein (α2-HS) (P<0.01).
    We concluded that determinations of individual bile acids were more sensitive indicater in diagnosing FH from AH.

  • Development of hepatic changes in HBs Ag carriers related to familial occurrence

    Clinical and pathological observations were performed in 246 subjects with persistent positive serum HBs Ag. They were divided into 3 groups, A: asymptomatic HBs Ag carrier (116 cases), B: HBs Ag carrier with abnormal liver function, firstly observed after long term follow-up study (52 cases), C: HBs Ag carrier with abnormal liver function during long term follow-up study (78 cases). Liver biopsies were performed in 66 cases of these carriers. HBV markers in their families were investigated in 97 subjects among 42 families.
    In group A 5 biopsied cases showed minimal histologic change in all, in group B 3 cases showed liver cirrhosis (LC) in 18 cases, and in group C 12 showed LC and 7 showed hepatocellular carcinoma (HCC) in 43 biopsied cases.
    Familial occurrence of HBs Ag carrier was significantly low in group A than in group B and C. Five cases with LC and HCC were histologically observed in 11 cases with familial occurrence, and one case showed LC in 19 cases without familial clustering.
    Seventeen cases were biopsied twice during 9 years. Histological development were observed in 4 of 5 cases with familial occurrence and 6 of 12 without familial occurrence.
    From these results it was suggested that a genetic background was related to the development of liver changes in HBs Ag carriers.

    The frequency of HBs antigen-antibody seroconversion and the effects of corticosteroids on the seroconversion were studied in lol HBs antigen-positive patients with hepatic diseases, whose diagnosis had been histologically confirmed and whom HBs antigen-antibody and e antigenantibody were followed up for over 2 years.
    HBs antigen disappeared in 5 (18.5%) of 27 users of corticosteroids, and HBs antibody developed in 3 of those 5 cases. On the other hand, spontaneous seroconversion from HBs antigen to antibody was seen in one (1.4%) of 74 subjects who had received no corticosteroids. This indicates that the disappearance of HBs antigen occurred more frequently in patients who were given corticosteroids as compared with the paptienta who were not given (p<0.01).
    A schedule for the administration of corticosteroids was as follows: an initial dosis of 40mg per day of prednisolone for a week, then 10mg reduction of daily dosis every week followed by 10mg 20mg every other day until titer of HBs antigen droped to 2 and the level of serum alanine aninotransferase reached below 100 units. No fulminant hepatic failure developed under the above condition.

    Results of lymphocyte stimulation test (LST), macrophage migration inhibition test (MIT) and leukocyte migration inhibition test (LIT) do not always correspond to those of other two. This study examines discrepancies using a new approved method.
    In order to remove the direct inhibitory effect on migrating cells, sensitized lymphocytes were cultured and stimulated by the antigen for a pulse duration (pulse-method). In using the pulsemethod migration inhibitory factors (MIF) were produced as similar as that used in the continuous stimulation by the antigen (continuous-method). MIF produced in greater amounts in culture mediums without serum (without-serum culture) than in culture mediums with serum (with-serum culture) in using the pulse-method. Seventy percent of positive LST cases were also positive in either MIT or LIT using with- and/or without-serum culture in using pulse-method. MIT tend to be positive in earlier stage following injury, and LST had been found positive even in later stage.

    In order to investigate whether antibody against alpha1-fetoprotein (AFP) could be used in the treatment of human hepatocellular carcinoma, the cytotoxicity of antibodies against AFP, human albumin, and specific liver cell membrane antigen was studied using hepatoblastoma derived culture cells as the target cell. Cytotoxicity tests performed consisted of two varieties: antibody dependent cellular cytotoxicity (ADCC) and complement-mediated cytotoxicity; as well as an immunofluorescent indirect technique. The reactive status of these antibodies was analysed. The results were: with the ADCC cytotoxicity test, anti-AFP showed 12.0±1.2% (m±SD) cytotoxicity, anti albumin 16.8±2.7%, and LMAb 40.1±3.1%. With the complement-mediated cytotoxicity test, anti-AFP showed 32.0±4.8% cytotoxicity, anti-albumin 34.2±5.0%, and LMAb 62.4±5.2%. The control was non-immune rabbit serum and showed a low level of 10.8±1.1%. With the immunofluorescent technique, AFP and albumin were stained in the cytoplasm of target cells in fixed material. With native cells, the cell membrane acted as a barrier to prevent staining. In contrast to this, the membranes of all target cells were stained regardless of whether LMAb was fixed or not. When human complement was used, Clq and C3 were demonstrated on the target cell membrane.

  • Fine structure of growth patterns

    Histological growth pattern of human hepatocellular carcinoma at tumor and non-tumor boundary can be classified into 2 types; sinusoidal and replacing. In the present study, 2 surgical cases of sinusoidal type and 6 surgical cases of replacing type were investigated electronmicroscopically.
    In the sinusoidal type, the tumor cells infiltrate in the sinusoid and/or Disse's space. Development of cell organelles, desmosome, junctional complex, and stud-like projections is poor.
    In the replacing type, the tumor cells infiltrate mainly along the Disse's space and degeneration of the endothelial cells of the sinusoid is predominant. The tumor cells grow as if they replace the non-cancerous hepatocytes. Cytoplasmic organelles, desmosome, junctional complex, and stud-like projection seem to develop more than those in the sinusoidal type.

    Ultrasonographic examinations were performed on 20 patients with hepatoma to evaluate the effect of transcatheter arterial embolization. Very strong high level echos were observed in the tumors after embolization. These echos were correlated with free air density in the embolized tumor observed by CT scan and may indicate gas formation in the tumors.
    The echographic patterns of the embolized tumors were variable but echo level of the tumor in effective cases were apt to decrease and in an uneffective case echo level increased. In relapsed cases echographic patterns became echogenic on follow up examinations.
    Our data indicate that ultrasonography is useful in evaluating and following the effect of the embolization in hepatomas because it is not invasive and be able to performed repeatedly.

    The patient was a 27-year-old man who was affected by jaundice in mid April 1981 with cenestpathy. The results of the examination revealed positivity of HBs-Ag and e-Ag and higher hemogram values of GOT 1170U, GPT of 1700U, and T-Bil. of 7.1mg/dl (D-Bil. of 4.5mg/dl). The biopsy of liver demonstrated the image of acute hepatitis. As the level of HBc-Ab was high since the bignning, acute onset of HBV-carrier was suspected. However, both the TTT and IgM levels were high and the IgM-Ab of HA-Ab was positive, this case was identified as hepatitis of Type A overlappingly affecting the HBV-carrier. His clinical course was very smooth, and the histological findings of liver were almost normal on the biopsy conducted on the 18th week.
    In this case, HBs-Ag in the hepatic tissue were distributed in cells isolated according to the first hepatic biopsy. On the second biopsy in the convalescent stage, positive cells were not perceived. But, HBs-Ag positive hepatic cells were observed on the biopsy conducted on the 18th week: these cells existed dispersedly and were stained intensively.

    Three cases of liver injury due to Tiopronin were reported. Jaundice was noticed on the 24th day in Case 1, 45 year-old female, on the 22nd day in Case 2, 48 year-old male, and on the 19th day in Case 3, 60 year-old female, after the oral administration of Tiopronin.
    Histological appearance of the liver biopsy revealed a lot of bile plugs in the central zone of the liver lobule in Case 1, but no bile plugs in Case 2 and 3. Large liver cells were scattered throughout the liver lobules, and contained multiple nuclei.
    In all three cases, the lymphocyte transformation test was positive when Tiopronin was added to the culture medium of the lymphocyte. The serum activities of leucine aminopeptidase and gamma glutamyl transpeptidase elevated as the serum bilirubin value decreased, and didn't return to normal level in 9 months after the onset of jaundice.

    A 67-year-old woman was admitted to our department on 7th July 1981 to determine the indication of surgical treatment for esophageal varices. She noticed abdominal fullness on 1975 and was pointed out liver dysfunction, ascites and esophageal varices at a hospital, and was followed up there with no symptom untill the hematemesis that occured 3 times on December 1980, March and June 1981. Slight hepatosplenomegaly was observed on the admission. Upper G-I series, liver scintigram, abdominal CT scan, selective celiac angiography and portogram during operation revealed various and complexed abnormalities as appointed above. Portal venous pressure was 31.3cm H2O. Liver biopsy specimen revealed liver fibrosis.
    In this case, various abnormalities in addition to A-P fistula make it complexed to speculate how the portal pressure elevated and to determine which is the main cause of portal hypertention.

    An autopsy case, 40 years old man, of Wilson's disease with hepatocellular carcinoma was presented. The patient noticed slurring of speech and intension tremor at age of 23 years. At age of 29 years, diagnosis of Wilson's disease was established by Keyser-Fleischer rings, liver cirrhosis, absence of plasma ceruloplasmine and etc., D-penicillamine and BAL therapy were continued for 11 years until the death.
    Histopathologically, liver showed coarse macronodular cirrhosis and hepatocellular carcinoma of diffuse type which occupied about 90% of the liver. In the brain, small necrotic foci were present in putamen and a lot of Alzheimer type II glia were widely distributed and a few Alzheimer type I glia and Opalski cells were found.
    Tissue copper concentration was determined by atomic absorption spectrophotometry, and the concentration of copper was increased in the liver, brain and kidney.
    Hepatocellular carcinoma is very rare in patients with Wilson's disease. There are only three cases reported previously in the world. The reason of rare occurrence of hepatocellular carcinoma in patients of Wilson's disease was discussed.
