    The incidence, clinical course and possible mechanisms of thrombocytopenia were studied in patients with acute hepatitis A.
    Thrombocytopenia was noted during 1-2 weeks after onset, and shortened platelet life time was found. Thrombocytopenia below 15×104/μl and 10×104/μl was recognized in 51% and 19% of cases, respectively.
    The platelet counts were negatively correlated with values of GOT, GPT and positively with total cholesterol and hepaplastin test.
    None of patients showed the evidence of bleeding tendency clinically or DIC determined by coagulofibrinolysis test. The platelet counts were negatively correlated with splenic size. High serum levels of IgA class and IgM class circulating immune complex (CIC) were observed during the period of thrombocytopenia. Furthermore the platelet counts were negatively correlated with the levels of platelet binding IgA (PB-IgA), but not with those of platelet binding IgG (PB-IgG).
    These results suggested that the degree of thrombocytopenia might represent severity of acute hepatitis A, and that thrombocytopenia might be produced by the enhanced destruction of platelets non specifically bound with IgA CIC in the reticuloendothelial system.

    To test the susceptibility of White Peking ducklings to in vivo transfection procedure, we injected 20μg of dimer DHBV DNA directly into the liver of ducklings of varying ages. (three 1-day-old, three 1-week-old and two 2-week-old ducklings.) In 1-day-old ducklings, serum DHBV DNA was detected in all three as early as one week, and complete virus replication was observed in the liver. In 1-week-old group, no serum DHBV DNA was detected throughout, but small amount of episomal DHBV DNA was detected in the liver in one of three ducklings. In 2-week-old group, DHBV DNA was not detected in serum or in the liver in any of two ducklings.
    Despite using the same amount of dimers, we observed marked difference in susceptibility to infection depending on the age of ducklings.
    Using this in vivo transfectioli procedure to one-day-old ducklings, it is possible to analyze the function of the DHBV genome efficiently.

    Male Wistar rats were paired feeding with standard diet, low protein diet, and with and without 36% ethanol. And the patterns of acid soluble nucleotides in the liver were analyzed. And the following results were obtained.
    1. The levels of triglycerides in the liver were higher in low protein-ethanol diet group, and fatty infiltration of hepatocytes, centrolobular fibrosis and pericellular fibrosis were observed.
    2. Cytosine triphosphates decreased significantly in this group. Adenine nucleotides were affected remarkedly by ethanol and dietary protein.
    3. In low protein-ethanol diet group, Atkinson's adenylate energy charge markedly decreased, and the redox state deviated markedly to reduction state. These factors may accelerate in turn the development of fatty change in hepatocytes and hepatic fibrosis.

    Thirty two autopsy cases with hepatocellular carcinoma (HCC) with sarcoma-like feature were analyzed and compared with those without during the past 17 years. The following clinical findings were suggestive of the sarcoma-like feature in HCC.
    1 Fever and evidence of lymphatic metastasis in the early stage of HCC with negative for serum AFP concentration.
    2 A metastatic-like appearance in abdominal angiogram with low serum AFP concentration.
    3 Progressive increase in tumor size with decreasing serum AFP levels after therapy.
    It is suspected that there exsists HCC with sarcoma-like feature among those with no response to the conventional therapy, especially transarterial embolization. We must consider the other therapeutic modalities for these cases on the definit diagnosis in thier life time.

    We studied Pre-S1/S2 polypeptides in 37 HBsAg positive serum samples of 35 chronic carriers by immunoblotting using anti-sera against synthetic peptides. Two bands at molecular weight of 42, 000 daltons (P42), and P39 were recognized by anti-Pre-S1 serum, and two bands of P36 and P33 by anti-Pre-S2 serum. Pre-S1 bands (P42/39) were found in 100% eAg positive sera and 46% of eAg negative sera. Pre-S2 bands (P36/33) were found in 100% of eAg positive sera and in 68% of eAg negative sera. In addition, Pre-S1 bands were found in 84% of HBV-DNA positve sera and 61% of HBV-DNA negative sera. Pre-S2 bands were found in 90% of HBV-DNA positive sera and in 72% of HBV-DNA negative sera. HBsAg titers of Pre-S1/S2 positive sera were higher than Pre-S1/S2 negative sera. These data suggested that the amount of Pre-S1/S2 polypeptides in seurm was greatly influenced by the amount of hepatitis B surface particles (small sphere and tubules), and not just by virions.

    The reticuloendothelial system (RES) constitutes an important part of cellular host defense in the removal of bacteria, immune complexes and microthrombi from the blood.
    The function of the RES, an estimation as a functional reserve in the liver is very important for the decision of an operative indication for hepatoma with cirrhosis. We examined the RES function, using pharmacokinetic techinique. The animal experiments by autoradiography showed that the 99mTc-phytate was uptaken into the RES. The elimination rate of the 99mTc-phytate from the systemic circulation in rats was analyzed by two-compartment open model.
    We could diagnose the RES function observating the change of the α-phase of the elimination curve in hepatectomized, cirrhotic and DIC rats.
    The functional test on the RES with 99mTc-phytate might be useful for one of the estimation of hepatic functional reserve in a large clinical field.

    The leukotriene B4 (LTB4) is derived from arachidonic acid via a lipoxigenase pathway and is a potent mediator of inflammation and allergy. In this study, to clarify the relation of leukotrienes to liver injury, we have examined the release of LTB4 from rat Kupffer cells in vitro.
    Kupffer cell suspensions (5×105 cells/ml) in modified Tyrode's solution incubated with the calcium ionophore A23187. LTB4 is assayed using combined technique of reverse-phase highperformance liquid chromatography and radioimmunoassay. When activated in vitro with the calcium ionophore A23187, Kupffer cells generated LTB4.
    Thus, our studies indicate that Kupffer cells also could generate LTB4 as well as polymorphonuclear leukocytes or monocytes. In addition, it is suggested that Kupffer cells may be able to modify inflammatory and immunologic events in liver tissue by the release of LTB4.

    Immunoglobulin (Ig) synthesis of peripheral blood lymphocytes (PBL) in vitro, suppressor activity for Ig synthesis of PBL and autologous mixed lymphocyte reaction (AMLR) were studied in 18 patients with primary biliary cirrthosis (PBC), in 15 patients with chronic active hepatitis type B (CAH-B) and 17 cases of healthy subjects as well. In PBC, spontaneous production of both IgG and IgM was higher than in normal control, and in CAH-B, only IgG production was higher than in healthy subjects. The synthesis of both IgG and IgM stimulated by PWM was almost the same as control. Suppressor activity for IgG production was reduced in PBC and CAH-B, but that for IgM was decreased in only PBC. So it was suggested that the decrease of suppressor T cell activity for IgM production might be involved in high level of serum IgM in PBC. Patients with PBC tended to be devided into high and low response group in AMLR and there were no relationships between the counts of AMLR and histological stages or clinical symptoms, but those counts tended to decrease with age. AMLR in CAH-B was almost the same as that in normal control.

    Extrahepatic lesions in the 13 autopsied cases with primary biliary cirrhosis (PBC) (Stage III or IV, according to Sheuer's classification) were studied histopathologically, and the following results were obtained. Chronic non-suppurative destructive cholangitis-like lesions were observed in the exocrine glands, the salivary excretory duct in 7 (54%), pancreatic excretory duct in 5 (38%) and the excretory duct of the esophageal mucous gland in 5 (38%). In the heart, rheumatic valvular disease-like lesions were observed in most of the cases, 12 (92%). In the esophagus, mucosal diturbance in the basal membrane associated with plasma cell infiltration was observed in 4 (31%). Fibrosis of inter-stitial tissues was observed in the lung of 6 (46%). Enlargement of abdominal lymphnode associated with plasma cell proliferation was observed in all cases. Above results indicated that the involvement histologically demonstrated that PBC induces lesions in a wide area over the whole body.

    Hepatocyte proliferation was studied in rats that had undergone a hepatic branch vagotomy. Nuclear chromatin structure, hepatic DNA synthesis and mean ploidy of nuclei were assessed by flow cytometric measurement of nucler DNA content. The results were as follows: 1) In hepatic branch vagotomy rats, nuclear chromatin structure was not uniform as compared to that of control rats (the hepatic branch of the vagus was intact). 2) The time course of DNA synthesis during liver regeneration was delayed by 3 hours in rats with hepatic branch vagotomy. 3) Changes in mean ploidy was also delayed as a result of delayed synthesis of DNA. Mean ploidy of vagotomy group was higher than that of control group at 168 hours after partial hepatectomy. The methodological justifications were fullydiscussed. It is concluded that the parasympathetic neural factors contributes to the synchronous progression of the process of hepatocyte proliferation. The results also suggest that the parasympathetic neural factors directly maintain nuclear chromatin structure in hepatocytes.

  • Cellular interactions between donor and recipient mice

    This report extends our previous study on experimetal hepatitis in C57BL/6 mice. Cellular immunity involved in the induction of liver injury in this model was studied by transfer of primed spleen cells from hepatitis donor mice to syngeneic recipient mice. Histological examination showed that primed spleen T-cells in the nylon wool adherent fraction were the essential requirement for the liver damage in the recipient mice. When the recipient mice were depleted of lymphocytes by whole body irradiation, transfer of primed spleen cells from hepatitis donor mice did not induce liver lesion in the lymphocyte-depleted mice. In a cell migration study using 51Cr labeled spleen cells, it was shown that infiltrating cells in the liver of recipient mice were mainly derived from recipient mice themselves. These data seem to indicate that cell-to-cell interaction between radiosensitive precursor cells of recipient mice and liver-antigen primed T-cells from hepatitis donor mice play an essential role in the induction of liver injury in the recipient mice.

    A 23-year-old man who is a carrier of hepatitis type B virus developed in October 1983 frequent episodes of unconsciousness, and admitted to our department in the following Novermber. High ammonia level in his serum and slight abnormality in his hepatic function were observed in laboratory data. EEG showed 2-3Hz triphasic wave and 5-6Hz theta wave even at clear consciousness. Laparoscopy for his liver and histology of his liver biopsy specimen showed the finding of chronic active hepatitis, but no cirrhosis. Abdominal angio-graphy revealed no abnormal shunt. Plasma aminogram showed that citrulline levels increased fifteen times more than that in normal. Hepatic enzymes concerning the urea cycle revealed a decreased levels of argininosuccinate synthetase with a titer of about one tenth of normal levels. Based on these results a diagnosis of citrullinemia was made. The restriction of his daily protein intake to 30g and oral administration of the specially-formulated milk (Meiji: 7925-A) for citrullinemia revealed a remarkable effect on his hyper-ammonemia.
    He was discharged in April 1984, and is in good condition attending to our out-patient clinic.
    The serial EEG study on this case was previously reported in "Rinsyo-noha" (27: 609-611, 1985). In the present case report, we made a report on details of the progress and the treatment.

    Two cases of primary biliary cirrhosis (PBC) with giant cell granuloma and intralobular granuloma formation were reported. Case 1 was a 35 year-old woman, who presented with bleeding from esophageal varices and splenomegaly. Anti-mitochondrial antibody (AMA) was strongly positive and elevated IgM (1599mg/dl) was noted. Histological examination of surgical biopsy revealed typical chronic non-suppurative desctructive cholangitis (CNSDC) and non-caseating granulomas with giant cells were noted in the portal zones. Case 2 was a 56 year-old woman, who was admitted to our hospital because of cholecystolithiasis. High level of alkaline phosphatase (344 IU/l) and positive AMA suggested the presence of PBC.
    Surgical biopsy showed typical CNSDC and well-defined epitheloid cell granulomas were noted adjacent to central vein as well as in the portal tracts. Of the twelve cases of PBC experienced in our laboratory two showed granulomas both in the portal area and in the intralobular parenchyma. A large specimen by surgical wedge biopsy and examination by serial section was thought to be useful in detecting granulomas in PBC.

    A 4-year treatment with 300mg/day ursodeoxycholic acid (UDCA) was evaluated in a 64 year-old woman with primary biliary cirrhosis (PBC). At the beginning of the study, liver histology revealed stage 1 of PBC. First, cholestyramine was administered. Pruritus was relieved but laboratory examinations were not affected. Then, UDCA 300mg/day in combination with cholestyramine was started 4 years ago. Total cholesterol (T. chol), alkaline phosphatase (Al-P) and glutamic oxalacetic transaminase (GOT) were improved but albumin declined gradually to below normal range. The second liver biopsy showed cirrhotic stage.
    UDCA apparently has choleretic effects but it remains unknown whether or not 300mg/day UDCA treatment has the potential to change the natural history of PBC.

    A 68-year-old male was admitted to our hospital because of prolonged high spiking fevers. On admission, marked hepatomegaly with elevations of the serum alkaline phosphatase (Al-P), leucine aminopeptidase (LAP) and γ-glutamyl transpeptidase (γ-GTP) was observed. Various secretions and tissues were studied by special stains and cultures for bacterial, mycobacterial and fungal agents without a single positive finding. Serological procedures performed were consistently unrevealing in the case. A liver biopsy revealed many non caseating small granulomas and focal necrosis, principally in the lobules. Administration of daily 50mg of prednisone was begun. Fever spikes to 39.2°C responded dramatically to prednisolone. Hepatomegaly and elevated serum Al-P, LAP, γ-GTP rapidly improved. The second biopay specimen of the liver after 2 months of steroid therapy showed decrease of granuloma and disappearance of focal necrosis. Immunohistochemically, the distribution of Coulter T-11 (and OKT-8) positive cells within the granuloma were sporadically in this case, unlike those cells in a ring-like manner around the granuloma as seen in sarcoidosis, tuberculosis and primary biliary cirrhosis.

    A 47-year-old male without any history of thorotrast injection, inhalation of vinyl chloride and another specific causes, was admitted to our hospital because of closer examination of dark urine and pretibial edema. On admission, the physical examination showed jaundice, hepatomegaly but could not demonstrated any vascularspider or palmar erythema.
    The computed tomography (CT) imaging of the whole liver showed a pattern with a mixture of high and low density area, and the low density area was irregularly and strongly enhanced. However, the findings of abdominal ultrasonography (US), magnetic resonance imaging (MRI), hepatic angiography and laparoscopy had indicated cavernous hemangioma which occupied the whole liver but could not demonstrate any malignant signs. However, the progression of jaundice and anemia was very rapid, that could clinically imply his hepatic tumor to be malignant. Finally he died in 103 days after admission, and the findings of the autopsy revealed of typical hepatic hemangiosarcoma.
