We treated 185 patients who had chronic hepatitis C with interferon (IFN) and studied the effect of interferon by means of Logistic regression analysis.
We defined good response as normalization of aminotransferase more than 6 months after termination of IFN and negative HCV-RNA at time of 6 months after termination of IFN.
Good response by means of IFN therapy is dependent to some factors, that is HCV Subtype. Histology, ways of IFN administration and HCV-RNA.
Cases of HCV K2 Subtype, CH2A, prolonged therapy of 8w continuous plus intermittent administration and less than 105 copy/ml of HCV-RNA are effective in treatment of IFN.

Endogenous digitalis-like substances (DLS) have inhibitory activity on Na+ K
+-ATPase as well as cross-immunoreaction with anti-digoxin antibodies. The present study was conducted to investigate the clinical significance of DLS in patients with acute hepatic failure (AHF).
Serum levels of DLS increased significantly in patients with AHF as compared with those with acute hepatits and acute hepatitis (severe type). These levels were elevated in parallel with the increasing severity of acute liver diseases. These levels were found to be particularly higher in patients with brain edema than in those without brain edema. Since the serum levels of DLS showed significant negative correlations with prothrombin time and the integrated CT number of the whole liver, and positive correlations with plasma levels of methionine and aromatic amino acids, it is conceivable that DLS levels change in accordance with the degree of hepatic failure.
Partial purification of DLS from the plasma of patients with AHF showed that DLS appeared in several different fractions on gel chromatography, and were also co-chromatographed with middle molecular weight substances which inhibit rat brain microsomal Na
+ K
+-ATPase activity. In conclusion, these results suggest that the determination of serum DLS is important to predict the development of fatal complications of AHF, particularly that of brain edema.

We studied on the relationship between hepatic oxygen metabolism (hepatic oxygen delivery (HOD), hepatic oxygen consumption (HOC), and hepatic oxygen extractional ratio (HOER) and hepatic venous oxygen saturation (HVSat)) after hepatic artery ligation using mongrel dogs. After all hepatic arterial supply was ligated, HOD, HOC, HOER and HVSat changed to 53%, 85%, 169%, 76% of their initial values respectively. We found the correlation between HOD and HVSat (r=0.8956, p<0.002, Y=23.2 LN(X)+7.75), and between HOER and HVSat (r=-0.8853, p<0.002, Y=-0.92X+86.61). Value of HVSat in hepatic artery ligation group was significantly lower than 60% which concidered the lowest point of normal liver. Therefore we concluded that it would be useful to measure hepatic venous oxygen saturation continuously for monitoring hepatic artery obstruction.

A 71-year-old woman was dianosed as luoid heatitis from the findings of high serum γ-lobulin level, positive serum autoantibodies (ANA, AENA, ARNP, LEcell) and histology of liver biopsy in 1988. In 1990, US and CT revealed a tumor with a size of 21×17mm in S
4 of the left kepatic lobe. In 1991, the tumor was enlarged and she died of hepatic coma. Hepatocellular carcinoma with Edmondson type II, a rare complication of lupoid hepatitis was confirmed by autopsy. Also she had a complication of pernicious anemia which had not been reported in Japan.

A case of fibrolamellar carcinoma of the liver (FCL) is presented. A 21-year-old Japanese man was admitted to our hospital with chief complaint of right hypochondralgia. Abdominal plain CT scan showed a low density tumor in the anterior and medial segment of the liver. A center of tumor was not enhanced by dynamic CT. Angiography revealed marked neovascularity. Anterior and medial segmentectomy of the liver was performed.
The resected hepatic specimen revealed a central stellate scar and calcifications. Histological appearance of resected tumor showed intermixing of broad fibrous strands and clusters of carcinoma cells which have eosinophilic cytoplasma. Fibrous strands showed lamellar stream. This tumor was diagnosed as a fibrolamellar carcinoma of the liver.
Imaging of FCL in the literature was discussed. Grossly and clinically, focal nodular hyperplasia (FNH) simulate with FCL. The appearance of the central scar may permit distinction of FCL from FNH, as that of FCL is poorly vascularized.
From this point, dynamic CT and MRI may be useful for the differential diagnosis between FCL and FNH.

A case of drug-induced liver damage caused by Sho-saiko-to in a 40-year-old male is presented. Laboratory tests revealed serum GOT to be 355, GPT 1205, LDH 535, ALP 990, LAP 382, γ-GTP 681 U/l, and eosinophils 2%. IgM-HA, HBsAg, and HCV-RNA (PCR) were negative. Tbe laparoscopic diagnosis was chronic inactive hepatitis with microscopic finding of non-specific reactive hepatitis. Electron microscopic examination revealed no abnormalities.
Lymphocyte stimulation test (LST) for Sho-saiko-to was negative, but when a small dose of Sho-saiko-to was administered to the patient, GOT, GPT, ALP, γ-GTP, eosinophils, and IgE increased. Thereafter, LST for each 4 components of Sho-saiko-to was found to be positive. These components were "Hange" (Pinelliae Tuber), "Ninjin" (Ginseng Radix), "Saiko" (Bupleuri Radix) and "Kanzo" (Glycyrrhizae Radix).
The final diagnosis of allergic drug-induced liver damage caused by Sho.saiko-to was established. To date, only seven cases of this entity have been reported in Japan. The value of performing LST for each component of the drug is emphasized.

We reported a rare case of carcinoma of the hepatic duct with the hepatic lobes, the left and the quadrate lobe, replaced with proliferated bile ductules. A 72-year-old female was admitted to the Osaka Medical College Hospital for jaundice. She died of hepato-renal failure three weeks after her admission. At autopsy, carcinoma arising in the left hepatic duct invaded the left portal vein, and the left hepatic duct and the left portal vein were obstructed. The hepatic artery was not affected. The left and the quadrate lobe, which are supplied from the left portal vein, were replaced with proliferated bile ductules. The configurations of the two lobes were well kept. We suppose that the proliferation of bile ductules in the present case relates to three factors, namely continuous stimulation for proliferation of bile ductules due to gradual obstruction of the left hepatic duct, gradual loss of hepatocytes caused by reduction of regionai portal venous flow following obstruction of the left portal vein, and maintenance of the hepatic arterial flow.

We report a case of extrahepatic portal obstruction, observed cavernous transformation. The patient was a 64-year-old woman and was admitted with accidental radial bone fracture. As she complained upper abdominal discomfort, routine test was performed to check up her health. Ultrasonography, Computed tomography and MR imaging revealed multiple tortous vessels at the porta hepatis. Angiography showed numerous hepatopetal collateral vessels around the obstructed portal vein. We diagnosed that she was a extrahepatic portal obstruction because of unknown etiology. She also had esophageal varices diagnsoed as RC (+), and we underwent a sclerotherapy four times. She recovered completely and had had no further bleeding for 33 months after treatment.
