    This study aimed to optimize two-stent treatment for left main coronary artery(LM)lesions and explore a novel approach in cardiovascular disease therapy. LM lesions present a significant threat to patient prognosis and require complex interventional techniques. We developed a new LM bifurcation lesion model that simulates cardiac motion, and investigated the standardization of techniques using 3D optical coherence tomography(OCT)guidance and optimal stent selection. This human-like simulation system provides a new methodology for pre-clinical assessment of treatment strategies, serving as an alternative to clinical trials for evaluating the safety and efficacy of the treatments. The results suggest that, given the current immaturity of two-stent procedures, the use of stent platforms with maximum expansion diameter, along with advanced imaging technologies such as 3D-OCT, can significantly improve treatment success rates. The evaluation of treatment strategies using this engineering-based model is expected to play a crucial role in the future development of medical technologies, improving the quality of care for patients and contributing to better outcomes in cardiovascular disease.

    This study aimed to determine the actual number of bacteria on the toothbrushes used by non-intubated emergency patients. Non-intubated the study included 27 non-intubated emergency patients. According to the patients’ status of performing activities of daily living, they were divided into the gel group, in which nurses provided oral care using oral gel, and the control group, in which patients independently performed oral care using mouthwash. Bacterial and adenosine triphosphate(ATP) levels were assessed before oral care, after brushing, after washing toothbrush, and after drying toothbrush. An interaction was found for ATP levels(F=10.3, p=0.002).After brushing, the ATP value was significantly higher in the control group(111683±89679RLU)than in the gel group(60786±48960RLU)(p=0.003).After cleaning the toothbrush, the ATP level was significantly lower in the control group(297±582RLU)than in the gel group(924±1098RLU)(p=0.004).After drying the toothbrush, the ATP levels were 42±48 and 48±54RLU in the control and gel groups, respectively, indicating that both groups remained clean.

    With advances in screening and treatment for children with genetic disorders, family care is important because genetic conditions can affect the genetic conditions of other family members. A literature review was conducted to clarify the practice of family care and coordination for parents, focusing on the couple subsystem.
    Using the Ichushi-Web, we searched for literature published since 2005 using the keywords“genetic condition”and“family”and excluded cases involving grief care. We identified 23 articles and 26 cases. Most were published before 2015, and there were few publications. A meta-study using the Suzuki/Watanabe Family Nursing Assessment/Support Model found that in family care for parents with a child with genetic condition, healthcare professionals' practices included[support for decision-making][support for acceptance of and attachment to their child][support for home parenting][family care in genetic counseling].
    From a family care perspective, the following were suggested as important points for supporting parents of children with genetic conditions:encouraging parents to understand the characteristics and individuality of their child's genetic condition;encouraging parents to express their feelings;considering the family life, including the child;and creating an environment that allows for multidisciplinary collaboration and sustained involvement.

    During cancer patients’ final stage of life, it is essential to provide nursing care that enables them to live their remaining days according to their wishes. However, delivering such care is often challenging. To address this, the authors developed a “real-image construction” method, which models cases of optimal care and structures the entire process in a clear and comprehensible manner. Additionally, a case study was developed to demonstrate how a patient's wishes were realized through the real-image construction method. The real-image construction method involves outlining facts, external factors, and foundational aspects, then combining these elements to establish a comprehensive framework for naming the care model. By summarizing individual cases using this approach, it becomes possible to identify the characteristics of a care model and apply these insights to similar cases. Additionally, as more models are developed in future, it will be possible to analyze them beyond the framework of facts, external factors, and foundational aspects. By categorizing models based on the similarity of their naming terms, new keywords critical to caregiving can be identified, leading to the creation of new, integrated care models.

    A 57-year-old man who was diagnosed with a gastrointestinal perforation during a plain abdominal CT scan performed to evaluate an acute abdomen and was urgently transferred to our hospital. The duodenal bulb had extensive ulceration and perforation in the anterior wall during the upper gastrointestinal endoscopy. Consequently, a laparoscopic-assisted closure of the perforation was performed with the round ligament of the liver. On postoperative day 5, the patient developed shock with massive melena. An urgent upper gastrointestinal endoscopy revealed a gastroduodenal artery pseudoaneurysm at the base of the ulcer on the posterior wall of the duodenum. After marking the duodenal mucosa adjacent to the pseudoaneurysm with a clip, the patient was taken to the angiography suite. Angiography revealed that pseudoaneurysm of the gastroduodenal artery was identified at the site corresponding to the clip, and hemostasis was achieved through coil embolization. The patient was discharged without any further bleeding, and the ulcer was healed on endoscopy two months later. We report a case of rupture of a pseudoaneurysm of the gastroduodenal artery into the duodenum after surgical closure of a perforated duodenal ulcer.
