  • Environmental Network Formation Model in a River-Lake Basin

    The activities of the community based environmental forum over the past 7 years to maintain the Hii River-Shinji Lake ecosystem were reviewed. The network formation model for environmental safeguard developed in local communities in Shimane and the action targets of a healthy city project were evaluated.
    1. Interaction between the development of theme and the increased participation of people from all walks of life.
    In 1991, the communities launched “The Chironomidae-outbreak counterplan” named “Shinji Lake Forum” with the participation of various classes of inhabitants around Shinji Lake, including fishermen, biologists and environmental health researchers. The participatory action spread from residents around Shinji Lake to other communities along Hii River, resulting in the formation of an enlarged community network and inter disciplinary research network. The action targets developed as follows:
    (1) the Chironomidae-outbreak counterplan, (2) water quality preservation of Shinji Lake, (3) community class for ecology, (4) a community network encompassing the river-lake basin, (5) a healthy city project aimed at cultural development.
    2. The objective of the healthy city project
    Now the participatory action is developing into the movement of “The Harmony of natural ecosystems”. With linkages between development and ecosystem protection, the healthy city project aimed at cultural development could be pursued in harmony with natural environment.
    3. “The network formation model that has gone through processes of opposition and sharing” and participatory action research
    In this paper we discuss the formation process of environmental safeguard network. At first, opposition structure such as the fishermen and the agriculturalists, the citizens and the administration, the upperstream area residents and the downstream area residents, was made obvious by making clear differences in interest concerning the environment and living well-being. And the common goals to achieve and roles individuals have to play become clear through the process of sharing a thought on the natural ecosystem and the process of recognizing the different standpoints again. The importance of the cooperation of residents, administrations and researchers, together with participatory action research using the network formation model, was emphasized in order to settle variously complicated problems of ecosystems.

  • Their Relationships to the Presence of Hepatic Disoder

    It is considered that hepatitis C virus (HCV) infection may be a pathogenic factor in cryoglobulinemia and glomerulonephritis. The purpose of this study was to determine whether there is any relation between the presence of hepatic disorder due to HCV infection and the association with cryoglobulinemia and urinary abnormalities, i. e. proteinuria and/or hematuria, in HCV-infected patients. Cryoglobulinemia was found in 11.4% of our HCV patients. However, seropositivity of cryoglobulin detected in all the patients was low. The prevalence of the association with cryoglobulinemia in patients with hepatic disorder was significantly higher than that in patients without hepatic disorder. Urinary abnormalities were found in 10.0% of the HCV patients. No significant difference was observed in the prevalence of urinary abnormalities between patients with and without hepatic disorder. In addition, there was no significant difference in the prevalence of urinary abnormalities between the association with and without cryoglobulinemia. These results suggest the close relation between the presence of hepatic disorder due to HCV infection and the association with cryoglobulinemia. However, renal involvement may have no relation with the presence of hepatic disorder and cryoglobulinemia.

    We studied the outcome of the surgical treatments, mainly on the walking ability, of the 113 femoral neck fractures of 113 limbs in the elderly, 70 and over, who were admitted in our hospital during the period between Nov. 1992 and Nov. 1996.
    Sixty-six of the 113 cases (60.0%) regained the pre-traumatic walking abilities. The rest of the cases, particularly patients over age 85, failed to make good recoveries because of advanced age aging, presence of various complications, types of fractures, and/or poor pre-traumatic walking abilities.
    Among various types of complications, dementia appeared as the number one factor in the poor recuperation. We, therefore, recommend that speech therapy as well as occupational therapy be used in the elderly with dementia to arrest further deterioration, and those without to prevent the occurrence of dementia during the perioperative periods with particular emphasis on the preoperative period while patients are still in bed with their legs under traction.
    The cases with inside neck fractures of Garden classification type III/IV recovered excellently, for which we applied bipolar arthroplasty (bipolar prosthesis with femoral stem). This is because the bipolar arthroplasty technique ensured much firmer fixation at the fracture site, earlier rising from bed, and earlier weight bearing than in the case of other operative techniques.
    The cases with the unstable lateral type of fractures, that are to be cured in osteosynthesis, and so treated by the Ender technique, did not recover well, because of infirm fixation at the fracture site, late rising and bearing times, and eventual concurrence and/or worsening of dementia.
    We, therefore, inclined to think that bipolar arthroplasty surgery would be more appropriate for the case of an unstable, complex type of fracture than the Ender fixation maneuver would be, because the former ensures the better outcome.

    A 56-year-old man, who was admitted with cerebral infarction in our hospital, was infected with MRSA, probably through the gastrointestinal endoscopy. Taking advantage of this episode, we reevaluated the procedure of cleaning and disinfection for endoscopes by bacteriological examination. Before encountering the MRSA case, we had routinely used a simple cleaning procedure. After an examination was over, an endoscope was wiped up with ethanol-soaked gauze, sucked an enzymatic detergent, water and positive soap solution through the biopsy-suction channel, and lastly was washed by the automated reprocessor (3-5 minutes for total process), if the patient was free of infectious diseases such as hepatitis B, C and syphilis. In this symple method, bacteria were detected in 10 out of 46 samples from after-cleaning endoscopes. The results were such that we adopted the new procedure for cleaning and disinfection as follows ; after wiping the surface of an endoscope by propanol-wet gauze, an endoscope was washed by neutral detergent manually, and the biopsy-suction channel was brushed up three times. Then, the endoscope was completely immersed in a 2% glutar aldehyde solution. In this procedure, no bacterium was detected in the samples from the endoscopes. However, as the concentration of glutar aldehyde rapidly decreased, it is necessary to use a test strip specific for the minimum effective concentration of glutar aldehyde frequently to monitor the potency of such solution

    We reviewed 15 cases of reconstruction of cervical esophagus by grafting with free jejunum, which has been performed in our hospital since 1987. Primary disorders were hypopharyngeal cancer (8 cases), cervical esophageal cancer (4 cases), mesopharygeal cancer (2 cases) and laryngeal cancer (1 case). Our team for this operation consisted of otolaryngologists, abdominal surgeons and a microvasucular surgeon. We took, on the average, 15 hours and a half for total operation and four hours and half for blood isolation of the jejunum. Postoperative troubles were as follows ; necrosis of the grafted jejunum (1 case), suture failure at the phyayngojejunal anastomosis (1 case), fistural formation at the pharyngoesophageal anastomosis (1 case), light dysphagia (3 cases), ileus (2 cases) and perforation of the jejunostoma (1 cace).

    We present a case in which nursing intervention could play a great role in improving the patients activity of daily living (ADL). A 81-years-old female with progressive Parkinsonian syndrome was admitted to our hospital with the chief complaint of dysphagia and weakness. As her dysphagia and weakness were very severe and drug treatment was totally in effective, tube feeding was begun. She was hardly expected to get out of the hospital. We tried to improve the ADL by helping her to conquer the difficulty in swallowing. First, oral hygiene and exercise of chewing were started to stimulate her to regain the will to eat. Second, we arranged her menu according to her ability of swallowing. The patient gradually had a desire for good, and her ADL was improved. Finally, she could eat gruel, and leave the hospital. Appetite is one of man's instinctive tendencies. We have learned that if patients got this kind of will, their ADL could be enhanced, and that nursing intervention could give the patients this will. We consider that nursing intervention is beneficial to patients with intractable neurological disease.

  • Year-by-Year Trends in Serum Total Cholesterol Values in Mass Health Screenings

    An attempt was made to correct variations in the measurement of total cholesterol levels in order to study year-by-year trends in the values of total cholesterol measured at our center as part of the mass health screening project.
    The sutudy covered the period from 1979 to 1996.
    In coming to grips with variations in the measurement, we used the month-specific means of in-house pooled serum, which is internal quality control serum. On the basis of. the in-house pooled serum, the use of which began in June 1993, we observed variations, while correcting variations between lots and computing the values of in-house pooled serum on one and the same level. Variations in the means of total cholesterol in the mass health screenings proved similar to those in the levels of measurement, suggesting that distortions in the measurement could be corrected. To compensate for the distortions, we computed conversion factors so that the vallues of the in-house pooled serum observed at one and the same level could be compatible with the levels of secondary certified reference material for the measurement of serum lipids and then used the factors to correct the age-specific means of total cholesterol recoreded in each year's mass health screenings.
    The year-by-year trends in the values of total cholesterol were such that the period of 18 years witnessed a rise of about 10mg/dl among men and about 6mg/dl among women in the age bracket of 29 and under, about 18-20mg/dl among men and about 16-17mg/dl among women in that of 30-59, and about 10-13mg/dl both among men and women in that of 60 and over. There were signs that the pace of rises from 1991 on was slower than during the 1981-1990 period.

  • The Effects of Seminar

    Today, well over 160, 000 patients undergo dialysis throughout Japan. The number of patients who are newly required to receive it is increasing year by year. Given that situation, our hospital has annually held a seminar since October 1994 for patients with renal diseases. The purpose is to slow the deterioration of renal function and to delay the introduction of dialysis by encouraging the patients to acquire a habit of taking high calory, low protein food.
    This seminar must have help the patients get more knowledgeable about morbidity and learn the benefit of low protein rice-based diet. Now that three years have passed since the opening of the course, we reviewed the outcome, comparing the effects of old and new restrict diets.
    A difference began to appear 9-2 months after the lst seminar between the seminar participants and the non-participants (control group) who received guidance only at the outpatient ward. The average rate of decrease in the serum creatine level of those participants who eated lowprotein rice was 0.029 and that of those participants who did not eat the restricted food was 0.166 (p<0.05), compared with 0.262 in the control group. We concluded that the difference is ascribable to the effect of the seminar. We would like to contribute to the well-being of the patients by enriching the content of the seminar and continuing to hold it.

    A case of multiple simultaneous intracerebral hematomas is presented. A 80-year-old man who had sudden onset of right hemiparesis and consciousness disturbance was admitted to our hospital. He had a long history of hypertension. A computed tomographic (CT) scan was performed immediately. It demonstrated a left thalamic hemorrhage and a left pontine hemorrhage. No abnormal vessel was recognized on enhanced CT scans, magnetic resonance (MR) images and MR angiography undertaken thereafter. Although he was treated conservatively, his consciousness was not improved after about 2 months of hospitalization. The occurrence of multiple simultaneous intracerebral hematomas is rare in this country. It develops only in a small number of patients who have intraparenchymal hemorrhage. The outcome is poor in the majority of these cases. The prognosis for survival is poor.

    Now that the measurement of Hb Aic has become routine in a diagnostic test for diabetes as part of a battery of tests in the physical checkup, we have come to know various factors cause Hb Aic values to fluctuate. Recently, we encountered a case in which the Hb Ale level is abnormally low. By isoelectric focusing, we identified Hb Malmo [β97 (FG4) His→G1n]. Some familial cases of Hb Malmo have been reported to be found in Sweden, Germany and Britain. It is first time that this case was discovered in Japan.

    A 75-year-old male visited Fuchu general hospital on foot because of possible acute mushroom poisoning. He had no symptom on admission. He twice ate some toxic mushrooms for lunch and for dinner on the previous day with his wife, who was found dead in her bed on the morning of his admission. A legally ordered autopsy was held on his wife, and a-amanitin was detected in her liver, brain and blood samples. Detection of a-amanitin in blood samples has never been reported in the literature. His liver and kidney failure rapidly progressed. He died on the 10th hospital day despite intensive treatment including hemodialysis and plasma exchange. On histological examination, his necropsy liver specimen revealed massive liver necrosis and new growth of pseudocholangioles, which apparently bore testimony to acute toxic mushroom poisoning.
