Abstract
Two patients with compensated liver cirrhosis showing liver failure triggered by acute pneumonia and acute cholecystitis, respectively, were treated with plasma exchange therapy.
One who manifested grade IV coma became alert after three exchanges and survived. The other who showed grade I coma and profound jaundice didn't recover from liver failure after as much as thirteen exchanges and died of renal failure.
Although plasma exchange therapy is not generally considered to be indicated for hepatic coma in decompensated liver cirrhosis, it is a treatment for choice for hepatic coma in compensated liver cirrhosis which is induced by curable complications. Even in this case, however, contineous high bilirubin level in spite of the treatment seems to indicate unfavorable prognosis.
